Who will do the Azure Service Management / user License assignment, DXC or Customer?
NOTE: DO NOT REMOVE the Global Admin Rights if DXC will manage a customer's service or subscription on their behalf. If Customer decided to remove the delegated Admin Rights, it’s required to send an email confirmation to DXC that they fully access and view all services or subscriptions before removing it, as DXC will no longer manage completely the service or subscription on their behalf.

IMPORTANT:
TERMS AND CONDITIONS (BY SUBMITTING THIS REG TEMPLATE YOU ARE AGREEING TO THE BELOW TERMS)
  1. To complete the processing of this application form, email approval from the WBS owner to accept all MS license costs which will be billed monthly must be emailed to cspservicedesk@dxc.com
  2. SLMS will charge a 2% Service fee for enabling the CSP contractual, infrastructure set up and operational solutions across DXC .
  3. SLMS will reconcile MS invoices and pay MS on your behalf & subsequently cross-charge the relevant costs. SLMS must pass all associated costs of the full Microsoft invoice (disputed and undisputed) to Account team for customer named above as SLMS will not be responsible for assuming any financial exposure.
  4. SLMS will send account team monthly reconciliation file from Microsoft that will indicate the license costs per customer. Azure is billed monthly in arrears and License based services are billed monthly in advance.

It is also understood between both parties that the A/c team assume full credit risk for customer payment and that WBS owner will not reject any cross-charges regardless of customer non-payment including non-payment for SLA issues with Microsoft, credits pending, amounts under dispute with Microsoft etc.
